Roaches coming out of the wall, sanitizing problems at Miami Beach restaurant
Sanitizing problems and roaches coming out of the wall helped a Miami Beach restaurant and event space fail inspection on Wednesday.
A routine inspection of the kosher 41 Miami, inside Tower 41 at 4101 Pine Tree Dr., turned up 23 violations, three of which were High Priority.

“Accumulation of black/green mold-like substance in the interior of the ice machine/bin,” pointedly the “inside of the ice machine on the catchall is soiled.”
The light fixtures in the kitchen held 18 dead roaches.
“Six live roaches coming out of the wall and the tile behind the dishwasher area in the kitchen.”
The can opener was “soiled.”
That was also the description for “the ceiling tiles and vents in the kitchen, prep and dishwasher areas.”
“Several ovens and a hot holding unit were soiled with grease and old food debris.”
In one place, “just cleaned and sanitized food pans were placed on the floor by an employee.” Elsewhere, “clean utensils were sorted in the dirty drawer of a black storage trolley.”
An employee prepped food without a beard guard. Another cut vegetables without a hair restraint.
“Trays and food containers were not sanitized as there was evidence the high temperature dishwasher” wasn’t reaching sanitizing temperature.
The inspector “observed two cycles on which the dishwasher reached a maximum temperature of 137 degrees on the gauge. The thermal label test (inspector’s and operator’s) strips also determined (the dishwasher) was not reaching the correct temperature for sanitizing.”
41 Miami passed re-inspection Thursday.
