Which is better to restore fullness in my face — fat grafting or fillers?

Q. My face feels like it is falling. I know I need volume, but I have mixed feelings about fat grafting versus temporary fillers. What are your thoughts?

A. This is a question I hear almost every day. The truth is that both fat grafting and fillers can be effective, but they behave very differently.

Fat grafting is attractive because it uses your own tissue. However, it’s inconsistent. Some of the fat may survive well, while a significant portion may be absorbed. Even under ideal conditions, fat survival can vary from one part of the face to another and from one patient to the next. As a result, additional procedures may be needed to maintain or refine the result.

This unpredictability is even more relevant with the widespread use of GLP-1 medications for weight loss. If a patient loses a significant amount of weight after fat grafting, much of the transferred fat can shrink or disappear, leading to loss of the very volume we worked to restore. On the other hand, if a patient later gains weight, the grafted fat can enlarge, sometimes making the face appear overly full.

Fillers offer a different approach. They are precise, consistent and highly reproducible. I know exactly how much volume I am placing, where it is going, and how it will shape the face. Results are immediate and controlled, which is critical when restoring facial structure rather than simply adding fullness.

When done thoughtfully, fillers do not change who you are. They simply restore what time has taken away in a balanced, natural, and reliable way.
