Q: I am a healthy 55 years old. I am considering having a breast lift and a tummy tuck in the next few months. I have 3 questions. Having a breast lift was my initial idea, but now I am unsure if I should consider a breast lift with a small implant. My breasts size are a full size C cup small D and I don't want to loose the size. When you do a breat lift do you loose some of the cup size? should I consider doing a small implant to maintain the size and fullness of the breast? My mother had breast cancer, and although I have not had a problem with my breasts and I have regular mamograms, in your opinion having an implant be more risky for someone like me? I am also exercising daily and eating very healthy and trying to loose around 30 pounds of weight, do you recomend to wait until I loose the weight before I do the surgeries? Lastly how do you find a doctor that specializes in these two procedures? most plastic surgeons seems to me that they do a whole bunch of procedures, but how do you find a plastic surgeon specializing in breast lifts and tummy tucks?

A: You certainly have alot of good questions. First let me tell you that my partner Dr Kelly specailizes in the operation that you want to do.You can reach him at 305-595-2969, Now to answer your questions. You should be examined by a physician because often times patients are not correct in their diagnosis. You will not necessarily loose a cup size if you did the surgery. You are not at added risk fo cancer as long as you continue yor mammography and follow up. And as far as you weight loss, if you do loose weight after the surgery you will probably look better. Q: Dr. Wolf, I am a healthy 54 year old woman. Eleven weeks ago I had blespharoplasty performed by a board certified plastic surgeon on my upper and lower lids. Now that the swelling has almost completely disappeared and the skin has smoothed out I am beginning to see what I consider to be issues with my left eye. On the upper inside corner is raised scar tissue. It is visible and the doctor recommends massaging the area with the tip of my finger. The lid on this eye also has a wrinkly appearance, while the right lid is perfectly smooth. On the outside corner of the left eye, skin covers the tip of my eye so that it is hidden. It looks like the skin was stitched too far over the eye. The skin pulls down in a straight line, giving a blunt look to the tip. It also gives the appearance of a small entrance at that corner. There are 2 lashes inside which cause some itchy discomfort. I have discussed these concerns with my plastic surgeon; his response was to tell me that A. He didn’t see the small hole at the corner of the eye. B. In his ten years of practice he has never done a revision. I understand that it takes six months for the healing process to complete its cycle. Is it too early to consider getting a second opinion from another plastic surgeon? I would like to add that I recently saw my eye doctor regarding a vision problem that has occurred since surgery. He did see the hole with the lashes inside. What is your opinion? Thank you, Susie

A: If your doctor tells you that he has never had to do a revision in his ten years of practise...he is either ignoring his problems or just plain not telling the truth. From what you are telling me, you should probably get a second opinion. It is not unusal to have wrinkling and mild assymetry initially after surgery. This however, usually improves over the next few months. I think that if you are not getting the feeling that your doctor understands your issues it is imperative to get that second opinion. Q: Hello Doctor. I went to my country to have a surgery. The doctor recommended to do a face liposuction and also take the Bichat Balls (Bolas de Bitach) I did what he told me however after my surgery, my acne scars became more visibles. Please let me know if there is any surgery to remove or decrease the apperance for the scars.

A: I DO NOT LIKE the operation where the fat pads are removed. I think what you need if volume to improve your scars. You could have fat injections or you could be injected with Restylane.Perlane.Sculptra or Radiessse. Your doctor would have to evaluate you.Submitted by aracely from lima - peru

Q: I had my nose done about 30 years ago. I am not happy with my nostrils from a side view I feel they need to be made smaller, sometimes you can see the insides and they appear red.I like my nose and shape otherwise. What is involved to correct this and is it worth fixing?

A: It is worth fixing if it bothers you enough to think about it. Now, the question is, can it be fixed. In order to determine that you would have to be evaluated either by pictures or in the flesh. Q: I would like to know what other type of surgery there is besides the tummy tuck to get rid of the kangaroo pouch after one has kids; that would not require the reconstruction of the belly button

A: If you are a real kangaroo there is really no other surgery. Just kidding. If you have a significant amount of skin above the belly button, the traditional tummy tuck is the way to go. If most of your skin is below the belly button you may be able to undergo surgery without moving the belly button( a mini tummy tuck) You would have to be evaluated in order to determine what the best course of action would be for you.Submitted by Zenny Hernandez from Indio, California

Q: Dear Dr Wolf. A little over a week ago I had lipo done on my abdomen & hips & breast enlargement surgery. While I am still a bit sore & uncomfortable, I am worried at what I have done! My stomach & hips looks worse than I started out with. I am bruised, which I realize will go away, but I am so swollen & puffy there that I dont notice any improvement at all. My doctor said all went well & it is fine! Also, after getting up in the morning my breasts hurt. Is all this "normal" & will it get better? How soon can I really expect to see improvement? Thank you

A: Patience is a virtue...you have alot of swelling and you are expected to have some discomfort!!We unfortunately live in a society of instant gratification. I think the biggest problem you have is that you may not have been informed of what to expect. We try to tell our patients everything that we think is important to know, before ,during and after surgery. The take home lesson here is that "YOU SHOULD ASK AS MANY QUESTIONS FROM YOUR SURGEON AS YOU CAN" that way you will know what to expect. If you are having pain and you have reddness,with unusual swelling you should contact your doctor immediately. If after a few weeks you are not content with the results or do not see some improvement talk with your surgeon about how you feel. When and if you do not get satisfaction you should get a second opinion. I think however, you will be fine if you just wait for the swelling to go away.Submitted by Gene Racond from Pembroke Pines, FL

Q: I am considering laser lower blepharoplasty, can you please let me know if this is the right and safest procedure, as I have asked my local doctor and he has told me there is no such think as laser blepharoplasty. A: When a doctor tells you its the greatest and only way ...run for the hills!! There is No advantage of doing a blepharoplasty with laser. About 10 years ago it was a great marketing tool for surgeons to get patients in for surgery. The most important objective is to get great results. Ask your surgeons if you could see pictures of their patients and if you could talk to a patient of theirs who you could see their results. Ask how many eye surgeries they do on a regular basis and approximately how many they have done. If you are not comfortable with your doctor there are plenty to choose from so keep searching. Q: I have had spider veins in my legs since I was a teenager. I have had a dermatologist do sclerotherapy with some success. Some areas improved, but others got worse. Is sclerotherapy still considered the most successful treatment for spider veins in the legs? Does success depend more on the doctor doing the procedure or the legs of the patient. I do lots of running and weights and my legs are in good health besides these unsightly veins. My mother had terrible varicose veins, so perhaps I should consider myself lucky. Any thoughts?

A: Like anything that requires experience...its all about the technique(ie the injector in this case). There are some lasers that can help and should be done by experts who do this all the time. Sclerotherapy is still the treatment of choice for spider veins. You should have an evaluation by a vascular surgeon to determine if you have any deep venous insufficiency. A: Yes we do offer a virtual web consultation. A virtual web consultation is a method where a patient can upload their pictures on a physician's web site and get an opinion if they are candidates for the type of surgery they want to have. Its a great way for patients who are out of the city to get an unbiased opinion from a doctor. Usually there is not fees associated with the patient consultion therefore the patient can have various opinions without leaving home. Naturally there are procedures which may not be condusive to this type of evaluation. Each doctor has their own preference.
