That inverted nipple could be a warning

Q. I hope you can help me. I have one nipple that goes in instead of sticking out. I am embarrassed to wear tight shirts and I just don't like it. Is there anything I can do about it?

A. An operation can repair your inverted nipple, but there are a few things to consider before you have surgery. If this is a new finding, it is important that you be evaluated by a surgeon as nipple retraction is sometimes one of the first visible signs of a breast cancer. You should also get a mammogram to make sure that everything is well with your breast.

Another important factor to consider is if you are young and plan to breast feed. There are two operations that can be performed to correct your inverted nipple; one spares the milk ducts, the other does not. Make sure you discuss your future plans with your plastic surgeon, including the risks and benefits of your operation.

The surgery can be done under local anesthesia. A small incision is made at the base of the nipple and either the milk ducts are cut or preserved, depending on your desires and on the degree of retraction you have -- your surgeon will determine which technique is best. The tissue is released and a ''purse string'' suture is tied to bring out the nipple. Recovery is usually uneventful and the results are very satisfying.
